Can we extrapolate the means by which to Make Disciples or the command to Make Disciples from verses other than the Great Commission? If so, which ones?

I think you would have to include a good hard look at how Jesus and Paul interacted with their disciples, wouldn't you? Jesus obviously used many effective means to challenge and train His disciples which are not mentioned in any of the great commission passages. Personally, I think one of the best "handbooks" for discipleship is the book of 1 Timothy.
The means are clearly revealed in the Beatitutes. In Matthew 5 Jesus lays out a sequence that all disciples find themselves going through as they learn to walk as His disciples.
